Chronic Disease Management

Managing chronic disease is a challenge, but you don’t have to go it alone. Our chronic disease management program has a team to help you. Big Sandy Health Care doctors, pharmacists, nurses, and community health workers are all part of a health care team. The team ensures that participants have the medical care and resources needed to maintain good health, such as medication management, advice about healthy eating, and keeping regular doctor appointments.


The chronic disease management program works with patients to help them understand and manage their chronic conditions. Participants receive a variety of services to improve their health. Though the program patients are educated about their conditions, learn about proper use of medicines, and even receive medical devices such as blood pressure and blood sugar monitors.


Our boots on the ground part of the team is the community health worker. Community Health Workers (CHWs) understand the needs of people in the their communities. They are a link between patients and the services they need. CHWs help patients by advocating for the health and social needs of patients. Through services such as home visits, medication delivery, and arranging support for a variety of other issues that affect health, a CHW becomes the patient’s key partner in improving health.
